1. Fill in the form.

2. Rename and save the form on your computer (in a place you can find it, like “My Documents”).

3. If it is a form requiring a signature (i.e., "Check Request Form" or "Time Sheet"), print out the form and fax or mail to the EMO Central Office. If it is a form that doesn't require a signature (i.e., "Event Planning Form"), you may attach the saved file to an e-mail message and send to mbush@emoregon.org.
