Restaurant Agreement

As a participating Dining Out For Life Portland location, we agree to:

- Contribute 10 percent of total sales (including food, beverage and liquor) during June 20-22, 2023, or $500 to Dining Out For Life Portland, an event to benefit the EMO HIV Day Center and OHSU Partnership Project.
- Allow EMO HIV Day Center and OHSU Partnership Project to use our business’ name in materials promoting Dining Out For Life Portland.
- Promote Dining Out For Life Portland by displaying event materials provided by Dining Out For Life Portland.
- Work Dining Out For Life Portland into our in-house, website and social media efforts to help drive diners to our venue during June 20-22, 2023.
- Inform wait staff, managers and other bar or restaurant personnel about Dining Out For Life Portland.
- Permit the EMO HIV Day Center and OHSU Partnership Project to place a volunteer Host and Ambassador at our location for Dining Out For Life Portland OR provide a staff member to act as ambassador and attend a training provided by Dining Out For Life Portland. Ambassadors welcome guests, explain the event, giveaway opportunities provided by Dining Out For Life Portland, and encourage diners to make an additional donation to Dining Out For Life Portland.
- Send or deliver a check for the total contribution payable to EMO HIV Day Center no later than Friday, September 23, 2022.

In exchange, the EMO HIV Day Center and OHSU Partnership Project will:

- Produce, utilize and distribute attractive, high-quality promotional materials for your use and for distribution.
- Orchestrate a visibility campaign to promote Dining Out For Life Portland via local print, radio, television and social media platforms.
- Promote your participation in Dining out For Life Portland on multiple social media platforms to encourage diners to continue to support your restaurant or bar on during June 20-22, 2023, and throughout the year.
